Physical Therapist (PT) / Occupational Therapist (OT) We are seeking dedicated and compassionate Physical Therapists and Occupational Therapists to join our team at Community Home Health Care in Fort Wayne, IN. In this role, you will provide exceptional rehabilitative care to patients in their homes, helping them improve their mobility, independence, and overall quality of life. Responsibilities

• Conduct thorough patient evaluations to develop individualized treatment plans.
• Deliver personalized therapy sessions focusing on improving strength, coordination, and daily living skills.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Monitor patient progress and adjust treatment plans as necessary.
• Educate patients and their families on therapeutic techniques and health maintenance.
• Maintain accurate and timely documentation in compliance with agency and regulatory standards.

Requirements

• Licensed Physical Therapist or Occupational Therapist in the state of Indiana.
• Strong clinical skills with a passion for home health care.
• Excellent communication and interpersonal abilities.
• Ability to work independently and manage time effectively in a home care setting.
• Valid driver’s license and reliable transportation.
